A comment on Q1 above. As we learn to solve problems using trail and error, we become good in recognizing patterns. Our parents, schooling system and society in general intereacts with us in a way that we become very good in reaching the conclusion based on patterns we have seen before instead of using the solid cause and effect. Any problem solving tools whether is A3, 8D or even Six Sigma, involves original cause and effect reasoning which we seem to lack or undervalue.
