Another classic &quot;biggest mistake&quot; in using A3s is failing to go to the gemba to discover what&#039;s really happening, and the real issues, instead doing an A3 at a distance, imagining one knows the reality of the problem and basing one&#039;s A3 on that (flawed) understanding.</description>
